The position is a work from home position, with travel required frequently locally and some overnight for marketing opportunities. Must have strong sales skills. SUMMARY Facilities the intake and referral process, supporting individual served placement, collecting and maintaining documentation, and working closely with the clinical and operational teams based on the needs of referred clients/individuals to facilitate appropriate placement and/or services   ESSENTIAL JOB FUNCTIONS To perform this job successfully, an individual must be able to satisfactorily perform each essential function listed below.  Strategic Growth Marketing Content: Implements defined marketing strategies in partnership with operations intended to reach individuals, families, guardians, counties, and managed care entities via methods such as social media, collateral, OneDay videos, community events, and sponsorships at the discretion of Intake Manager if applicable Community Engagement: Participates in local sponsorships, community events, and other activities to promote organizational visibility and partnerships Liaison to Funders: Develops ongoing relationships with funding agencies and MCOs, providing timely communication, credentialing, and issue resolution in partnership with Intake Manager if applicable   Referral Intake & Data Entry Referral Ownership: Manages the referral process by collaborating with external case managers and internal clinical and operations teams to ensure appropriate individual placement and access to services; proactively escalates referral challenges to the intake manager or local leadership as needed Referral Data Entry: Collects, verifies, and enters all referral-related data, including individual demographics, insurance/payment information, medical assessments, to support placement activities Referral Documentation: Oversees the timely completion of intake documentation; identifies and resolves any issues that may prevent successful data entry or delayed required paperwork   Census & Periodic Service Management Referral Process Management: Ensures timely completion on key steps in the referral process, including outreach, assessments, eligibility verification, funding and placement. Escalates to operational leadership as needed for referral holds, closures, or other needs Service Management Meetings: Facilitates consistent execution of meetings, including weekly referral reviews, monthly in-depth discharge reviews, at-risk/service preservation calls, and quarterly service management reviews   Service Delivery & Quality Stakeholder Communication: Provides clear, respectful, and timely communication with individuals served, families, and payors regarding service planning, admissions, and other updates to achieve our goal of being the provider of choice for these partners Service Accountability: Aligns service commitments with program capabilities to deliver on customer expectations Customer Feedback & Resolution: Analyzes customer complaints and service issues with operations, ensuring successful resolution to maintain consistent customer satisfaction   Reporting & Insights Data Monitoring: Reviews state or regional census and periodic data and trends regularly pending assigned scope, including with intake and state leadership and growth director       Other Emergency Coverage: Assists with emergency coverage and crisis intervention support when needed Performs other duties and projects as required   SUPERVISORY RESPONSIBILITIES None   Minimum Knowledge and Skills required by the Job The requirements listed below are representative of the knowledge, skill, and/or abilities required to perform the job.    Education and Experience:  Bachelor's degree in human services or related field Two years’ experience preferred An equivalent combination of education and experience which provides proficiency in the areas of responsibility listed above may be substituted for the above education and experience requirements   Certificates, Licenses, and Registrations: None required.   Key Metrics: State or Region Revenue, EBITDA Budgeted Census and Admits Periodic Service Admits & Utilization Referrals to Achieve Future Metrics Time to Initial Outreach Time to Assessment Time to Site Visit Time to Placement OneDay Videos   Other Requirements: Regional travel as needed; estimated 25% depending on market scope (site visits, community events, OneDay videos)   Physical Requirements: Light work.  Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects.  If the use of arm and/or leg controls requires exertion of forces greater than that for sedentary work and the worker sits most of the time, the job is rated for light work.
