About the Role

We areĀ seekingĀ a compassionate and patient-focusedĀ Part-Time Nurse Case ManagerĀ to support patients, healthcare providers, and clinic partners through a specialized Patient Support Program (PSP).Ā 

As a key member of the care team, you will help patients navigate their treatment journey by providing education,Ā careĀ coordination, and ongoing support. You will collaborate with physicians, clinics, pharmacies, and internal stakeholders toĀ facilitateĀ timelyĀ access to treatment and help deliver an exceptional patient experience.Ā 

This is a part-time position requiring approximatelyĀ 2.5 days per week. Scheduled hours must fall within the program's operatingĀ hours ofĀ Monday through Friday, 8:00 a.m. to 8:00 p.m. EST.Ā

What You'll DoĀ 

  • Serve as a primary point of contact for patients enrolled in the Patient Support Program.
  • Provide educationĀ regardingĀ prescribed therapies, treatment expectations, medication administration, and adherence strategies.Ā 
  • Support patient enrollment and onboarding activities.Ā 
  • Coordinate care and communicate with physicians, clinic staff, specialty pharmacies, and other healthcare professionals.Ā 
  • AssistĀ patients in navigating access to treatment and reimbursement processes.Ā 
  • Conduct patient assessments and follow-up activities as required by the program.
  • Monitor patient progress and provide ongoing support throughout the treatment journey.
  • MaintainĀ accurateĀ andĀ timelyĀ documentation within program systems.
  • IdentifyĀ and escalate clinical or operational concerns asĀ appropriate.
  • Contribute to quality improvement initiatives and program success measures.
  • Build andĀ maintainĀ positive working relationships with healthcare providers and clinic partners.Ā 

Basic RequirementsĀ 

  • Active and unrestricted nursing license inĀ Alberta and/or British ColumbiaĀ (RN, RPN/LPN, or equivalent nursing designation).Ā 
  • Diploma, degree, or equivalent combination of education and experience consistent with theĀ P1 Professional Career Framework.Ā 
  • Strong verbal and written communication skills.Ā 
  • ProficiencyĀ using Microsoft Office and electronic documentation systems.Ā 
  • Ability to work approximatelyĀ 2.5 days per weekĀ within program operating hours ofĀ 8:00 a.m. to 8:00 p.m. EST, Monday through Friday.Ā 
  • Ability to travel to clinics and healthcare sites throughout Alberta and British Columbia asĀ required.Ā 
  • Excellent organizational skills and attention to detail.Ā

Preferred Skills & ExperienceĀ 

  • Experience working within aĀ Patient Support Program (PSP).Ā 
  • PriorĀ NurseĀ Case Management experience.Ā 
  • Experience supporting patients receiving specialty medications or biologic therapies.Ā 
  • Knowledge of reimbursement, patient access, or specialty pharmacy processes.Ā 
  • Experience providing patient education and clinical support.Ā 
  • Experience collaborating with physicians, specialty clinics, and multidisciplinary healthcare teams.Ā 
  • Familiarity with chronic disease management andĀ treatmentĀ adherence programs.Ā 
  • AdditionalĀ active nursing licenses in Canadian provinces.Ā

Travel, Work Environment & Physical RequirementsĀ 

  • Primarily remote/home-based role with travel asĀ required.Ā 
  • Travel to clinics, healthcare facilities, and stakeholder meetings throughout Alberta and British Columbia.Ā 
  • Valid driver's license and access to reliable transportation may beĀ requiredĀ based on territory coverage.Ā 
  • Must be able to work independently in a remote environment while effectively managing competing priorities.Ā 
  • Occasional lifting and transportation of educational or program materials may beĀ required.Ā
