The Senior Support Service Worker performs complex and highly sensitive paraprofessional work in support of the Health Care Program for Children in Foster Care (HCPCFC), with guidance provided by the Senior Public Health Nurse. This position plays a critical role in supporting children and youth in out-of-home placement, including justice involved youth, access Medi-Cal, navigate the health care system and effectively utilize Medi-Cal benefits. It also supports the County’s CalAIM and Enhanced Care Management (ECM) initiatives through inter-departmental coordination and community outreach.
The Senior Support Service Worker supports care coordination activities, assists foster youth and caregivers in navigating Medi-Cal and managed care systems, monitors compliance with state and county health mandates, and maintains accurate health documentation, including the Health Education Passport (HEP). The role requires close collaboration with Public Health Nurses, Child Welfare Workers, Probation Officers, county departments, community-based organizations, healthcare providers, and caregivers to ensure continuity of care, particularly during placement changes and transitions. The position also supports program development by delivering trainings, tracking data, monitoring performance measures and ensuring compliance with state required reporting, while maintaining strict confidentiality in compliance with HIPAA and County policies.
HYBRID SCHEDULE
The Health and Human Services Department has embraced a hybrid work schedule that considers that some duties of this position may be performed remotely. However, the successful candidate must be able to report to County facilities for meetings, work assignments, and/or disaster service work upon short notice, as needed and deemed appropriate. During the initial probationary period, the successful candidate may be required to work primarily onsite before establishing a hybrid work schedule.
The highly qualified candidate excels at coordination and problem-solving across multiple systems and settings, including public health, child welfare, juvenile probation, behavioral health, and community partners. They are technologically proficient, comfortable tracking performance measures and maintaining detailed records, and able to support evolving CalAIM and ECM initiatives with flexibility and resourcefulness.
