What you’ll do:
The Population Health Administrator (PHA) is a senior population health and value-based care leader responsible for the strategic design, execution, and oversight of Population Health Management (PHM) initiatives across all organizational sites. The role drives performance improvement through data analytics, cross-functional collaboration, and alignment of clinical, quality, and financial objectives.
The PHA leads enterprise-wide efforts related to value-based contracting, HEDIS and CMS quality measures, health equity initiatives, Health Information Exchange (HIE) participation, and population-level reporting. This role partners closely with Executive Leadership, Information Technology, Clinical Operations, and external health plan partners to ensure regulatory compliance, accreditation readiness, and continuous improvement in patient outcomes *IN OFFICE HOURS REQUIRED * YOUR APPLICATION WILL BE REJECTED IF YOU ARE NOT A RESIDENT OF THE PHOENIX, AZ AREA.
Essential Responsibilities:
- Lead the design, implementation, and daily execution of Population Health Management programs aligned with Medicaid, Managed Care, and Value-Based Service (VBS) contracts.
- Direct provider performance initiatives to improve quality outcomes, contract performance, and patient care delivery across the organization
- Serve as the organizational subject matter expert for population health, quality measurement, and value-based care strategies
- Coordinate logistics for population health projects (scheduling internal meetings, tracking action items, maintaining project plans).
- Leverage multiple internal and external data sources to identify care gaps, quality performance trends, and opportunities for system-level improvement.
- Translate complex health plan, HEDIS, CMS CQM, and contract reports into actionable workflows and targeted interventions for clinical teams
- Monitor, track, and report progress on population health initiatives to leadership using
- Collaborate with Executive Leadership, IT, Clinical Operations, and external partners to implement system-wide process improvements
- Serve as a liaison for Health Plans and other population health entities, ensuring compliance with all quality and value-based contract requirements.
- Serve as project lead and primary contact for Health Information Exchange (HIE) and onboarding, maintenance, and reporting requirements
- Maintain user guides, tip sheets, and process maps for internal tools (EHR registries, data exports, BI dashboards).
- Develop and deliver trainings, presentations, and guidance to support adoption of population health workflows and quality initiatives
What you’ll provide:
- Bachelor’s degree in Behavioral Health, Healthcare Administration, or similar related field required.
- Minimum of 3 years of experience in a behavioral health environment.
- Advanced knowledge in population health management, value-based care, and quality measurement (HEDIS, CMS CQMs, NCQA)
- High attention to detail and strong organization skills are essential. Experience using electronic health care records.
- Excellent interpersonal skills, capable of confidently and effectively presenting and speaking.
- Able to work independently, collaboratively and proactively to accomplish work goals.
- Proficient in Microsoft Office Suite and managing projects through a full cycle process.
- Proficient in basic date analysis, reporting to include spreadsheets, charts and diagrams.
- If driving while on agency time or on agency business, a valid Arizona driver license and appropriate liability insurance and approval for liability coverage with EMPACT - SPC’s liability insurance carrier is required. At time of hire, a 3-year state driving record with no major infractions or excessive driving violations/tickets is required.
- Must have a Fingerprint clearance card or be eligible to obtain one.
