What We’re Looking For

 

Trillium Health Resources has a career opening for a Supervisor, Population Health Clinical Support, to join our team! The Supervisor, Population Health Clinical Support is responsible for providing administrative oversight, clinical supervision, and operational support within the Clinical Services Organization. The Supervisor plays a key role in maintaining compliance with regulatory, contractual, and accreditation standards, while driving initiatives that strengthen integrated physical and behavioral health care management. This includes oversight of high-acuity case collaboration, workflow development to close care gaps, and targeted member engagement strategies.

 

On a typical day, you might:

 

  • Serve as the primary operational liaison to the Vice President, Clinical Operations and Program Oversight for all Clinical Oversight Team activities, including providing regular updates, reports, recommendations and support the VP in operational planning, staffing allocation, and process optimization within the Clinical Services Organization. 
  • Oversee, monitor and evaluate activities of Population Health Oversight Clinicians and Auditing Clinicians to ensure adherence to workflows, performance expectations, and quality standards. Conduct performance reviews as required. 
  • Develop and implement ongoing Wellness/Health Promotion Campaigns. 
  • Conduct analysis on ongoing case audits, monitor program outcomes, and ensure documentation meets regulatory, contractual, and accreditation requirements to ensure performance measures and benchmarks are consistently met.
  • Provide direction and consultation for high-acuity and complex cases, facilitating coordinated responses across disciplines and collaborating with staff to maintain compliance with accreditation standards.

Qualifications

 

Required:

  • Master’s degree and three (3) years of experience providing care management, case management, or care coordination to the MH/SU/BH population; OR 
  • Licensure as a Registered Nurse (RN) and three (3) years of experience providing care management, case management, or care coordination to the MH/SU/BH population.
  • Fully licensed in the State of North Carolina as one of the following: LCSW, LMFT, LCAS, LCMHC, LPA or RN. If applying with an Associate’s or Bachelor’s degree, you must be fully licensed in the state of North Carolina as an RN.
  • Must have a valid driver’s license.
  • Must reside within North Carolina.
  • Must be able to travel within catchment as required.

 

Preferred:

  • Degree in Human Services field.
  • Three (3) or more years of supervisory experience in a managed care, LME/MCO, or integrated care environment.
  • Compliance and clinical program development experience in a fast-paced environment.
  • Program evaluation, workflow development, and quality/performance monitoring experience.
